The complete record,
minus the busywork

Pokta turns each rheumatology consultation into a structured, complete clinical record. You approve every note; nothing leaves without your sign-off. The same record reaches where it belongs: your registry, and a plan your patient finally understands.

From a consultation to a complete record. Rheumatology first.

A complete record doesn’t fit in 15 minutes.

What a complete record demands

  • Diagnosis and disease activity
  • Treatment and its adjustments
  • Indices and follow-up studies
  • What the clinical registry asks for
  • And nothing left out

The protocols exist. The time to apply them doesn’t.

What a 15-minute visit leaves behind

  • The record stays half-finished
  • The registry gets filled in late, or never
  • Data that matters stays in the note, unstructured
  • And the patient leaves with a fraction of it

With Pokta the record is complete, and you approve every detail

Each consultation becomes a structured, complete record. You review and approve it, and the same record reaches your registry and a plan your patient can follow. Less time on work that doesn’t pay you.

What each consultation turns into

A complete record, ready for your registry

Each consultation is structured into a complete clinical record (diagnosis, disease activity, treatment, follow-up), with anything missing flagged before you close the note. You approve; the record is ready for wherever it needs to go.

A plan your patient actually follows

The same approved record becomes a plain-language plan your patient understands, with reminders and follow-up between visits.

The right data, the right way

The approved record goes where it needs to (your registry, your record) and nowhere it should not.

  • Reliable
  • Private
  • Compliant

How Pokta works

One loop, with a single non-negotiable step: the clinician approves before anything leaves.

  1. 01

    Capture

Record or upload the consultation. No new workflow to learn.

  2. 02

    Structure

Pokta organizes it into a structured clinical record.

  3. 03

    Complete

It flags what a complete record needs and is still missing, in time to act.

  4. 04

    Approve

The clinician reviews and signs off. Nothing leaves without this.

  5. 05

    Deliver

The complete record reaches your registry, and a plan your patient can follow.

Nothing reaches a patient until the clinician approves it

Pokta drafts. The clinician decides. Every plan and every message is signed off before it is shared, so the record always reflects real clinical intent.

  • Drafts arrive ready to review, not blank pages to fill.
  • You stay in control. Nothing is sent without your sign-off.
  • Built for rheumatology first, grounded in real clinical work.

For patients

Understand your treatment, and keep it close for whenever you need it.

  • Your plan in plain language, without the jargon.
  • Instructions and follow-ups you can revisit anytime.
  • Leave every visit knowing exactly what happens next.

What makes Pokta different

Complete, not just faithful

A perfect transcript of a consultation where a question never got asked is still an incomplete record. Pokta catches what is missing, not just what was said.

Clinician-approved by design

Pokta drafts; the clinician decides. The approval gate is built into the loop, not bolted on after.

Built for rheumatology

We start where the clinical foundations are strongest (EULAR/ACR and national guidelines), not in a generic tool for everything.

Where you already work

Chat-native, in the app you already use. No portal to remember, no new app to download.

Built with clinicians who use it

Pokta gives my patients a plan they actually understand, and it keeps the final word with me. Nothing goes out that I have not approved.

Questions, answered plainly

Does Pokta replace the clinician’s judgment?

No. Pokta drafts the plan and the patient-facing version; the clinician reviews and approves everything before it reaches a patient. The judgment stays with the clinician.

How is patient data protected?

Health data is treated as the most sensitive category. Consent is explicit and scope-limited, and we do not process real patient data without an approved data-handling posture in place.

Which specialties does Pokta support?

Pokta starts with rheumatology, the surface where the clinical foundations are strongest, and expands from there.

Where do patients see their plan?

In a durable record they can return to anytime, written in language they can actually follow.

Is Pokta available now?
